An internal audit should do more than confirm that documents exist. It should test whether an important process works as intended, expose risk early and give managers evidence they can act on.

Start with the purpose of the process

A checklist can confirm that a procedure contains the expected headings. It cannot show whether people follow it, whether handovers work or whether customers receive a consistent outcome.

Begin by asking what the process is meant to achieve, which risks could prevent that outcome and what evidence would demonstrate effective control.

Choose scope according to risk

Auditing every process with the same frequency wastes attention. Use complaints, changes, previous findings, supplier performance and business priorities to decide where a closer look will add value.

Follow evidence across the real workflow

Interview the people doing the work, observe a sample, review records and compare practice with the organisation’s own requirements. Evidence should be sufficient to support the finding without turning the audit into surveillance.

Independence matters. ISO and IAF guidance notes that audits should be carried out by competent people who can provide an objective view of the activity.

Write findings people can use

A good finding identifies the requirement, the evidence and the gap. It avoids blame and describes the operational effect. Separate nonconformity from observation and improvement opportunity so priorities remain clear.

Agree an owner, due date and proportionate action. Correcting one record is not enough when the cause sits in training, workflow design or unclear responsibility.

Close the loop

Follow up to confirm that action was completed and effective. Review repeated themes across audits rather than treating each finding as isolated.

• Did the action address the cause?
• Has the process outcome improved?
• Did the change create another risk?
• What should be shared elsewhere?

The strongest audit programme becomes an early-warning and learning system, not an annual paperwork event.
